Step 2: Answer “Which of the following best describes the reason for your dispute?” with “My card was stolen.”
Step 3: Click the Continue button
Step 4: Locate and select the disputed transaction(s) or click Manually add the transaction to add a transaction to the claim, click the Continuebutton
Step 5: Answer the additional questions, then click the Continuebutton

In response to “When was your card stolen?” use the calendar tool to select two days prior to the earliest disputed transaction date
In response to “Where was the card stolen?” select “Office” from the drop-down
Answer “Do you have additional details about how your card was stolen?” with “Yes”
Enter the following in the “Additional Details” freeform box: I left my wallet in my backpack. My backpack was at my desk while I went out to a work lunch, but it was stolen and not in the office’s lost-and-found.
In response to “State” select “CA” from the drop-down
Answer “Was anything else stolen?” with “Yes”
Enter the following in the “Please provide additional details on what else was stolen.” freeform box: My wallet was stolen including my Quavo card, Visa card, $20 cash, and my ID. I had a USB-C phone charger and my Fitbit Charge 3 in my backpack, too.
Answer “Have you ever authorized another person to use your card?” with “No”
Answer “Have you ever shared your Personal Identification Number (PIN)?” with “No”
Answer “Would you like us to send you a replacement card?” with “No”
Answer “Do you have any additional information to provide about this dispute?” with “No”
Step 6: On the Summary screen, review the information, then click the Submitbutton
